Membership Forms

Here are links to the two forms that the Sons of the Revolution uses for new members. There is a standard form for all new members and a short form for family members of current members of the Virginia Society. These forms are in Adobe .pdf format. If you don't have Adobe Acrobat Reader installed on your computer, please click on the Adobe Reader icon to install the free version. Right click on the link and save the Adobe .pdf file to your hard drive. You can then open the file with Adobe Acrobat Reader.

Instructions for completing the forms are included with each form.

Please note:

ALL APPLICATIONS MUST BE TYPEWRITTEN OR HAND PRINTED AND SUBMITTED IN DUPLICATE. (only one copy of documentary proofs need be submitted.)

If you need additional help in researching your family tree to trace it back to your Revolutionary War ancestor, the Virginia Society has a list of Genealogists for your convenience. Right click on the link and save the Word file to your hard drive. You can then open it with Microsoft Word.
